From the North or South
-Take I-294 South (Tri-State Tollway) to I-90 East (Kennedy Expressway), and take exit 82C at Austin Avenue.
-Travel south 2.9 miles on Austin Avenue to Addison Street, and turn left.
-On Addison Street, travel east 0.5 miles to Community First Medical Center.
-Visitor parking is located on the north side of Addison Street.
From the East (Downtown)
-Take I-90 West (Kennedy Expressway) to exit 45A for Addison Street.
-Travel west 2.6 miles to Community First Medical Center at Addison Street and Central Avenue
-Visitor parking is located on the north side of Addison Street.
From the West (O'Hare)
-Take I-90 East (Kennedy Expressway) to exit 82C for Austin Avenue.
-Travel south for 2.9 miles on Austin Avenue to Addison Street, and then turn left.
-On Addison Street, travel 0.4 miles east to Community First Medical Center.
-Visitor parking is located on the north side of Addison Street.

7:30 a.m. – 4:00p.m. Monday through Friday.
Area of Service
We operate within a 7 mile radius of Community First Medical Center. Our area of service consists of the following boundaries: North :Devon Street, South: North Avenue, East: Kimball, West: Cumberland.
How To Schedule the Shuttle Service
To schedule the Shuttle, call 773-794-5088. Please have the following information available when you call.
-The patient’s name, address and phone number.
-The appointment date, time and location.
Criteria for All Riders
-Patients must be ambulatory or, if utilizing a support device, must be stable enough to walk unassisted.
-Patients who are oxygen dependent may ride if they are able to self-administer their oxygen, and if their units can be strapped or tied to ensure passenger safety.
-Patients must travel within the designated boundaries.
Costs
$5.00 – Round-trip to the Medical Center (within a 7-mile radius)
$4.00 – Each companion traveling with patient(s). (A requirement for patients scheduling some procedures).
For your convenience, ride cards can be purchased from the Community First Medical Center Cashier office located on the 1st floor. The drivers carry NO CASH.
Additional Information
On the day of your appointment, we ask that you be ready to depart when the shuttle driver arrives. Our goal is to ensure that you arrive for your appointment at or before your scheduled time.
The Shuttle Service schedule fills up quickly. We ask that you provide as much notice as possible, preferably 48-72 hours. We will make every attempt to schedule all rides and will do so in the order in which requests are made.
